I am aware that in accordance with the requirements of the Federal Law dated July 27, 2006 No. 152-ФЗ «On Personal Data», including the provisions of paragraphs 2-4 of Part 1 of Article 6 of the Federal Law, will be processed the personal data I provide, including full name, date of birth, phone numbers, email addresses, as well as other data provided by me.
At the same time, I confirm my consent to processing and performance by MEDIPAL LLC (hereinafter referred to as the Operator) of the following actions with my personal data: collection, recording, systematization, accumulation, storage, extraction, clarification, updating, change, use, depersonalization, distribution (including transfer to third parties), blocking, destruction. This consent does not establish a time limit for the processing of personal data.
Personal data is provided by me and processed for the purpose of working with information on the safety of medicines (drugs) for scientific, medical and preventive purposes, including, but not limited to, for the purposes of pharmacovigilance, as well as complaints, appeals, reports of violations, conflicts of interest of the Operator and other information about violations of local regulations and current legislation of the Russian Federation.
I agree that the Operator has the right to process my personal data by entering it into electronic databases or databases on tangible media (databases processed without the use of computer equipment), inclusion in lists (registers) and reporting forms provided for by the current legislation of the Russian Federation for the purposes specified above, as well as by other means provided for by the current legislation of the Russian Federation.
MEDIPAL LLC undertakes not to transfer the personal data received to third parties, except when providing personal data at the request of authorized public authorities of the Russian Federation only on the grounds and in the manner prescribed by Russian legislation, as well as to third parties (representatives of legal entities, including organizations-holders of registration certificates of medicines) with which MEDIPAL LLC has concluded appropriate contracts for services in pharmacovigilance (including other services for collection of information).
I reserve the right to withdraw my consent by drawing up an appropriate written document, including by means of computer technology, certifying such document with an electronic signature, which may be sent by me to the Operator by registered mail with return receipt requested, or delivered in person against a signature to the representative of the Operator. If I receive my written application to withdraw my consent to the processing of personal data, the Operator shall be obliged to stop processing my personal data within the time period necessary to complete its use, and, if necessary, to immediately notify the third parties specified in this document in case such actions are not contrary to the current legislation of the Russian Federation. The validity of this consent to the processing of personal data in the absence of withdrawal of this consent on my part is not limited.
By expressing this consent to the processing of personal data, I acknowledge and confirm that the consent is informed, given freely, willingly and in my own interest, after familiarizing myself with the purposes of personal data processing. I am also aware that I am responsible for the accuracy and truthfulness of the data I provide and do not object to the verification of this information.
